Needleless high-pressure water-jet elevation creates a submucosal fluid cushion and lifts up the tumor-bearing mucosa. Unlike the healthy surrounding mucosa, the adenoma does not change color during elevation when using sodium chloride fluid with indigocarmine dye. This makes it easy to identify the lesion.
Incision and dissection
The mucosa is incised at a sufficiently safe distance from the adenoma, beginning aborally. Using the swiftCOAG mode, the submucosa can be optimally dissected. The blood vessels are exposed well and temporarily compressed through elevation and can be selectively coagulated.
The fluid cushion can be replenished at any time. This maintains the protective effect and distinction function over the entire course of resection.
Vessels are coagulated during resection using the swiftCOAG mode. The pathologist can make an optimal histological assessment of the en-bloc resected tissue.
